It's time to nominate your sporting hero of the year - that someone who's made such a difference to your team, club, players or sport.

The BBC North East Sports Awards recognise contributions across all elements of sport – from coaches to volunteers to elite performers and teams.

The wealth of talent in the region and the unbounded passion for sport never ceases to amaze. Do you know of someone without whom your sport would not be the same? Someone who goes that extra mile, who embodies true sporting passion. If you do, now is the time to say "THANK YOU!"

This year’s BBC North East Sports Awards will take place on Wednesday 25^th November at the Silksworth Community Pool, tennis and Wellness Centre. BBC in-conjunction with Sport England and Sunderland City Council are organising the event.

Your 'sporting hero(s)' can be nominated for these awards via the down-loadable form below. Nominations must be in by 23rd October 2015.
